Locate an Enrollment Center and Make an Appointment

Register with Identogo.com through the above site. Take your receipt and a valid photo id to the fingerprinting site. (Call the site first to be sure they are still open.) Once you have registered and had your prints scanned, please submit your registration id number (UE ID number) to the HR department.

Apply for a Criminal Record Check

1. Click Submit a New Record Check.
2. Read the Terms and Conditions and click Accept.
3. Select Individual Request and click Continue.
4. The reason for the request is Employment.  Fill in the rest of the form and hit next.
5. Verify your information and if everything is correct, click Proceed.
6. Enter your information again and click Submit this Request.
7. Verify your information and click Submit.
8. Click Checkout.
9. Enter your credit card information and click Pay with Credit Card.
10. Print two copies of the certification page, one to keep and one to give to Human Resources for your personnel file.

This secure website is provided for individuals who want to have their Pennsylvania Child Abuse History Clearance processed online. The Pennsylvania Child Abuse History Clearance Check will provide the applicant information as to whether or not they are listed in the Pennsylvania statewide database as a perpetrator of child abuse.
